Proof of ID with full name and address:
- Arizona driver license
- Arizona non-operating ID card
- Tribal enrollment card or tribal ID with photo
- Any federal, state, or local government ID
If you don't have one of the above, bring TWO of:
- Arizona vehicle registration
- Arizona vehicle insurance card
- Property tax statement of your residence
- Additional options
